The Chronic Disease Self-Management Program is a 6-week program that was developed by Stanford University, and it is aimed to help individuals with chronic conditions learn how to manage and improve their own health, while reducing health care costs. The program will be taught by two trained facilitators from the Middlesex County Office of Health Services and Hackensack Meridian Health.
Dates for the six week program are: Tuesdays, October 8, 15, 22, 29; Wednesday, November 6, and Tuesday, November 12.
Open to adults 18 and up. Participation is limited to 15 and registration is required.
